Vor seiner Laufbahn als Filmkomponist spielte er als Trompeter beim Oakland und San Francisco Symphonie Orchester sowie bei der San Francisco Oper. 
Schon bald zog es ihn von der klassischen Musik in Richtung Pop und Jazz, wo er unter anderem für die Beach Boys und Van Morrison spielte. 
Seine ersten veröffentlichtenAlben bestanden vornehmlich aus Jazz, spätere Arbeiten wandten sich mehrheitlich in Richtung Pop.

Beim Film debütierte er als Komponist 1983 mit seiner Musik zu „Never Cry Wolf“ (83). Danach folgten noch einige weitere Filmproduktionen, u.a. „Mrs. Soffel“ (84), „The Times of Harvey Milk“ (84), „Trouble in Mind“ (85), „The Hitcher“ (86), „Made in Heaven“ (87), „The Moderns“ (88) und „The Beast of War“ (88). 

Mark Isham erkannte die grosse Chance, mit der Filmmusik ganz unterschiedliche Musikstile anwenden zu können. In den 90er Jahren richtete er sein Augenmerk hauptsächlich auf das Schreiben von Filmsoundtracks. 

In den kommenden Jahren entstanden eindrückliche Arbeiten zu Filmen wie „Point Break“ (91), „Little Man Tate“ (91), „Billy Bathgate“ (91), „A River Runs Through It“ (92), „Of Mice and Men“ (92), „Romeo Is Bleeding“ (93), „Short Cuts“ (93), „Mrs. Parker and the Vicious Circle“ (94), „Timecop“ (94), „Nell“ (94), „Quiz Show“ (94), „The Net“ (95), "Fly Away Home" (96), "Blade" (98) und "At First Sight" (99).

Zu seinen jüngsten filmischen Arbeiten zählen "Men of Honor" (00), "Hard Ball" (01), "Moonlight Mile" (02), "Spartan" (04),"Twisted" (04), "Running Scared" (06), "The Black Dahlia" (06), "Lions for Lambs" (07) und "The Mist" (07).

Mark Isham erhielt zahlreiche Auszeichnungen, darunter zwei Grammy-Nominationen, eine Golden-Globe-Nomination, drei Emmy-Nominationen und den Gewinn des Emmy 1997 sowie eine Oscar-Nomination.
